The named items are always dropped by the same bosses so I compiled a list of what bosses have a chance to drop what named items. I'll be filling it in as I find more items and figure out the names of some of the quests.

Note: There are only nightmare versions of the weapon mods. The other items drop at the same level in nightmare.
