Nope, not everyone. Many people living downtown use a rideshare program (sort of like a cheap rental car) when they need to leave the city. It's just not convinient or cheap to keep a car in the city so most people that move here end up getting rid of their cars pretty quickly.
The Bay Area Rapid Transit can take people to the east bay (Berkeley, Oakland, etc.) or to the airport or anywhere within a few miles.
The main problem is that San Francisco is 7 square miles and has nearly 750,000 residents. This doesn't count the number of tourists or commuters that come here daily. With that many people there just isn't any room for cars.
